Outpatient Physical Therapist
Assignment Duration
This is a 16-week contract position.
Schedule
This position follows a 5x8.0 schedule, requiring 40 hours per week.
Job Description
The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on patient needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients and ensures the design of functional treatment programs to facilitate effective community reintegration. The therapist serves as an educator for patients, families, students, and coworkers while adhering to the organization's Mission Statement and Core Values.
Responsibilities
  • Conduct comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals.
  • Document care provided in a clear, concise, accurate, and timely manner, ensuring compliance with state and federal guidelines.
  • Provide clinical supervision and a positive learning environment for students during their program.
  • Develop and pursue an individualized professional development plan and engage in continuing education.
  • Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
  • Participate in organizational performance improvement initiatives and follow through with action plans.
Required Experience / Certifications / Licensure
  • Degree in Physical Therapy from an Accredited School.
  • Minimum of 2 years of outpatient clinical experience.
  • Current CPR certification.
  • Active and applicable license for the state where work will be performed.
