Job description

Outpatient Physical Therapist

This is a 13-week contract position.

5x8.0 Shift

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs allowing for effective return to community activities.

The therapist serves as an educator of patients, families, students, coworkers, and themselves, performing professional duties associated with patient care, development of discipline-specific therapy services, and design of program delivery. This position also emphasizes compliance with the organization's Mission Statement, Core Values, and Code of Conduct, focusing on patient and employee safety.

Responsibilities:

  • Completes comprehensive evaluations of assigned patients and develops appropriate treatment plans and goals.
  • Documents medical records in a clear, concise, accurate, and timely manner.
  • Ensures care is provided under signed, valid physician orders and within insurance approval.
  • Participates in student programs as assigned, providing supervision and evaluations.
  • Creates an individualized professional development plan with clinical management.
  • Adheres to discipline-specific State Practice Act and maintains professional licensure.
  • Implements organizational performance improvement initiatives.

Required Experience / Certifications / Licensure:

  • Degree in Physical Therapy from an Accredited School.
  • 2 years of outpatient clinical experience preferred; no new grads.
  • CPR Certification required.
