Job Title: Social Worker (PACE) – Temporary
Location: Garden Grove, CA
Job Type: Full-Time | Temporary
Schedule: Monday–Friday | 8:00 AM – 4:30 PM
Salary Range: $68,015 – $108,824 annually ($32.70 – $40.31/hr)

About the Role

We are seeking a compassionate and experienced Social Worker to support participants enrolled in the PACE (Program of All-Inclusive Care for the Elderly) program. This role focuses on providing comprehensive care management, psychosocial support, discharge planning, and coordination of community resources for elderly participants with complex medical and behavioral health needs.

Key Responsibilities
  • Conduct psychosocial assessments and care planning
  • Participate in Interdisciplinary Team (IDT) meetings and participant assessments
  • Coordinate care management services for elderly participants
  • Support discharge planning and transitions of care
  • Provide counseling, crisis intervention, and participant advocacy
  • Connect participants and families with community resources and support services
  • Assist with enrollment support and care coordination activities
  • Maintain accurate medical documentation and reports
  • Participate in quality improvement initiatives and case conferences
  • Conduct home visits and travel within Orange County as needed
Minimum Qualifications
  • Master’s degree in Social Work from an accredited college/university
  • Minimum 2 years of experience in a health-related field
  • At least 1 year of experience working with the elderly population within the last 3 years
  • CPR and First Aid Certification (or ability to obtain within 6 months)
  • Valid driver’s license, reliable transportation, and current auto insurance
Preferred Qualifications
  • Case management experience with the elderly population
  • Bilingual skills preferred (Spanish, Vietnamese, Korean, Chinese, Arabic, Farsi, or Russian)
Required Skills
  • Strong communication and interpersonal skills
  • Care coordination and case management expertise
  • Crisis intervention and psychosocial assessment experience
  • Ability to work independently in a fast-paced environment
  • Proficiency with Microsoft Office and documentation systems
