The PACE Driver is responsible for providing safe transportation to participants in a timely, competent manner and communicating information, relevant to the plan of care, with the interdisciplinary team (IDT). This position reports directly to the PACE Transport Supervisor. PACE drivers are required to demonstrate the knowledge and skills necessary, to provide service to frail elderly participants according to assigned responsibilities and (PACE Program) standards. The driver has direct interaction with caregivers and contracted partners, while conducting business on behalf of the Franciscan Senior Health and Wellness (FSHW) Program of All-Inclusive Care for the Elderly (PACE) program. The PACE offers seniors and their families the care, nutrition, rehabilitation, transportation, and supportive services they need to remain healthy so that they can live in their own home.

WHAT YOU CAN EXPECT
  • Practice Hours: Monday - Friday, 8:00 a.m. - 5:00 p.m.
  • No Weekends, Evenings, or Holidays
  • Escort and transport patients in a timely and safe manner.
  • Inspect and maintain vehicle supplies and equipment and ensure that vehicles are in proper working condition.
  • Assist passengers on and off the vehicle in a safe and courteous manner.
  • Follow established schedules, adjust schedules to meet unanticipated changes, and follow the safest and most efficient pickup and delivery routes.
  • Take part in participant care planning and contribute to notations in medical record.
  • Report important information to proper staff of Franciscan Alliance PACE daily regarding any changes, progress, or unusual events concerning any participant.
  • Conduct daily checks of PACE vehicles prior to transporting participants.

QUALIFICATIONS
  • High School Diploma/GED - Required
  • 1 year Experience with a frail or elderly population - Required
  • 1 year Experience in a PACE program - Preferred
  • 1 year Transportation Experience- Preferred
  • Driver's License - Required
  • Auto Insurance - Required
  • Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required
