Job Description
Highland Hospital is seeking an experienced RN to join the team as a Stroke Navigator.
The Stroke Navigator-RN provides expert nursing leadership for stroke patients across the continuum of care, ensuring adherence to evidence-based practice and regulatory requirements. This role performs advanced neurologic assessment, supports timely clinical decision-making, and promotes high-quality, coordinated stroke care in alignment with national standards and certification expectations.
The Stroke Navigator-RN leads stroke quality monitoring activities, including chart review, data abstraction, documentation audits, and performance tracking aligned with Get With The Guidelines (GWTG) measures. The role identifies compliance gaps, provides real-time follow-up, reinforces evidence-based practice, and drives continuous quality improvement initiatives. The Stroke Navigator-RN serves as a clinical resource to nursing and provider teams, promoting consistent adherence to stroke standards of care.
The Stroke Navigator-RN also supports stroke program operations, staff education, regulatory readiness, and interdisciplinary collaboration to advance patient outcomes and sustain certification excellence.
Salary Range:
$90,000-$122,870
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Job Requirements
  1. Education: BSN required; MSN preferred
  2. Experience: Minimum of 2 years of clinical nursing experience, 5 years preferred
  3. License/Certification Required: Current NYS RN license required, Basic Life Support (BLS) certification required
  4. Skills: Stroke care, quality improvement, or data abstraction experience preferred. Strong ability to perform chart review, data abstraction, and performance metric tracking. Proficiency with electronic medical records and data management tools. Excellent communication, organization, and interdisciplinary collaboration skills
