What You'll Do
Deliver Exceptional Patient Care
Conduct comprehensive nursing assessments and develop individualized plans of care that promote health, independence, and quality of life.
Clinical Excellence
Perform skilled nursing visits, including medication management, wound care, disease management education, and patient/caregiver instruction.
Care Coordination
Monitor patient progress, communicate changes in condition, and collaborate with physicians, therapists, and interdisciplinary team members to ensure optimal outcomes.
Documentation
Complete timely and accurate OASIS assessments, nursing notes, care plans, and other required documentation through our EMR system.
Patient Advocacy

Requirements
What You Bring
Credentials
Graduate of an accredited nursing program with a current, unrestricted Registered Nurse (RN) license in the state of practice.
Experience
Minimum of 1 year of nursing experience required; Home Health experience preferred.
Reliability
Valid driver's license, current auto insurance, and dependable transportation.
Tech-Savvy
Comfort with electronic documentation, mobile communication, and EMR systems.
Clinical Knowledge
Strong assessment, critical thinking, and patient education skills with the ability to work independently in a home health setting.
