) Vertus HealthCare is a leading home health agency committed to delivering compassionate, high-quality, patient-centered care. We are currently seeking a dedicated Registered Nurse (RN) to join our interdisciplinary team and make a meaningful impact in patients' lives.
Position Summary:
The Home Health Registered Nurse (RN) is responsible for providing skilled nursing care, coordinating patient services, and ensuring continuous assessment of patients' physical, medical, psychosocial, emotional, and spiritual needs. This role plays a key part in developing and implementing individualized plans of care for patients in a home setting.
Patient Care & Case Management:
  • Perform admissions, skilled nursing visits, and follow-up care for home health patients
  • Provide ongoing case management and coordinate patient care
  • Conduct comprehensive assessments and develop individualized care plans

Care Planning & Coordination:
  • Collaborate with physicians to develop and implement treatment plans
  • Revise care plans based on patient progress and ongoing assessments
  • Coordinate care with interdisciplinary team members (PT, OT, HHAs, and other providers)

Clinical Duties:
  • Deliver hands-on nursing care and patient education in accordance with physician orders
  • Monitor patient condition and respond to changes appropriately
  • Ensure continuity and quality of care

Communication & Documentation:
  • Communicate patient status, progress, and concerns to physicians and Clinical Manager
  • Maintain accurate, timely, and compliant documentation
  • Supervise and ensure proper documentation by home health aides and support staff

Requirements:
  • Associate or Bachelor's degree in Nursing
  • Active and unrestricted Florida RN license
  • Current CPR certification
  • Minimum 1 year of nursing experience (preferred)
  • Strong clinical background (Med-Surg, ICU, ER, or acute care experience highly valued)
  • Home health experience preferred but not required
  • Excellent assessment, communication, and organizational skills
