Description:

The Hybrid Certified Pharmacy Technician & Medication Historian supports inpatient pharmacy operations while ensuring accurate patient medication histories overnight. This role assists pharmacists with medication preparation, dispensing, compounding, inventory management, and maintaining a compliant pharmacy environment. Additionally, it involves interviewing patients or caregivers to document complete medication histories, verifying information with external sources, and collaborating with the care team to enhance patient safety and quality of care.

This is a full-time, 40 hours per week night shift position with 10-hour shifts Monday through Thursday.

The Hybrid Certified Pharmacy Technician & Medication Historian will have inpatient technician duties as well as being responsible for completing patient medication histories overnight. As a certified pharmacy inpatient technician, you will assist the pharmacist in assuring timely and accurate provision of medications to patients. Routine technician functions are performed under the direct supervision of a pharmacist. These functions may include unit dose dispensing, sterile and non-sterile compounding, inventory control, appropriate record keeping, and use of pharmacy equipment and automated technology. As a medication historian, you will interview patients or their caregivers and document an accurate medication history including any allergies and the dose, route, frequency, and last dose taken of medications.

? What you will do:

  • Support safe, timely, and accurate medication preparation, dispensing, and distribution under pharmacist supervision

  • Perform sterile and non-sterile compounding, including chemotherapy, in accordance with regulatory and USP standards

  • Maintain automated dispensing cabinets, including restocking, troubleshooting, and inventory management

  • Manage medication inventory, including notifying purchaser of ordering needs, stock rotation, and expiration monitoring

  • Maintain a clean, organized, and compliant pharmacy environment

  • Obtain accurate and complete medication histories through patient, caregiver, and family interviews

  • Document prescription and OTC medications, including dose, route, frequency, and adherence

  • Verify medication histories with external sources (pharmacies, provider offices) or communicate follow-up needs to day shift historian

  • Identify discrepancies and escalate to pharmacists as needed

  • Collaborate with pharmacists, nurses, and providers to support optimal patient care

  • Promote a culture of safety by following policies and procedures and reporting safety concerns as needed

Minimum Qualifications:

  • Education: High school graduation or equivalent

  • Licensure / Certification: Approved Pharmacy Technician Certification. Active registration as a Certified Tech with Ohio State Board of Pharmacy.

  • Experience: Previous pharmacy technician and/or medication historian experience is desirable

  • Effective written and verbal communication skills

  • Aptitude in pharmacy mathematics
