Job Summary:
The Transplant Outreach Coordinator (TOC) works in collaboration with referring providers and dialysis centers to generate and support transplant referrals. The TOC serves as a liaison between the transplant team and referring providers and dialysis centers to build and maintain positive relations, increase referrals, provide education and information between the groups, and to mitigate and/or resolve issues, problems, and challenges. The TOC works with solid organ transplant (SOT) service line leadership to use data to identify opportunities for program growth, supports satellite clinic operations, and provides health-related education to the community. The TOC will support the providers and operations staff of the SOT service line on all outreach initiatives and strengthen relationships through collaboration with faculty, staff and community partners.
EXPERIENCE DESCRIPTION:
Minimum of 3 years as a Transplant Coordinator is required. Previous outreach, marketing or sales experience is preferred.
EDUCATION DESCRIPTION:
Associate's degree in Nursing with a minimum of 10 years of relevant experience is required or Bachelor's degree in Nursing with a minimum of 3 years of transplant coordinator experience is required. Bachelor's degree in Nursing is preferred.
SPECIAL SKILLS DESCRIPTION:
High degree of organizational skills and excellent communication skills; ability to work independently and take programmatic initiative. Knowledge of data systems, outcomes data and other clinical performance data. Ability to travel to locations throughout Wisconsin and Northern Illinois. Bilingual in Spanish is preferred.
LICENSURE DESCRIPTION:
Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). Certified Clinical Transplant Coordinator (CCTC) certification, Certified Clinical Transplant Nurse (CCTN), or Acute/Critical Care Nurse (CCRN) is preferred.
