Job Summary

Homeside Rehab is seeking a compassionate and dedicated Physical Therapist Assistant (PTA) or Occupational Therapy Assistant (OTA) to provide home-based therapy services in Pound Ridge, NY 10576.

This is an excellent opportunity for a therapy professional looking for flexible scheduling, competitive compensation, and one-on-one patient care while working with a supportive clinical and administrative team.

Job Responsibilities
  • Provide skilled therapy services under the supervision and direction of a licensed Physical Therapist or Occupational Therapist

  • Carry out established plans of care and treatment interventions based on patient needs and goals

  • Assist patients with improving mobility, strength, functional independence, activities of daily living, and overall quality of life

  • Monitor patient progress and communicate changes or concerns to the supervising therapist

  • Maintain accurate and timely documentation of all patient care in accordance with regulatory and company requirements

  • Collaborate with supervising therapists and the clinical team to ensure coordinated and effective treatment

  • Communicate professionally with office staff, patients, and caregivers regarding scheduling and patient care

Job Qualifications
  • Active New York State PTA or OTA license/certification — required
