Job description

Homeside Rehab is seeking compassionate and motivated Physical Therapist Assistants (PTAs) and Occupational Therapy Assistants (OTAs) to provide one-on-one home-based therapy services throughout Sullivan County, NY, including Swan Lake, Fallsburg, Woodridge, and Monticello.

At Homeside Rehab, we provide high-quality rehabilitation services in the comfort of our patients' homes. Our therapists enjoy flexible scheduling, competitive compensation, and the opportunity to make a meaningful difference by helping patients improve their mobility, independence, and overall quality of life.

Job Responsibilities
  • Provide skilled PT or OT treatment services under the direction and supervision of a licensed Physical Therapist or Occupational Therapist

  • Carry out established plans of care and individualized treatment programs

  • Provide therapeutic exercises, functional activities, mobility training, and other interventions within your professional scope of practice

  • Monitor patient progress and communicate changes or concerns to the supervising therapist

  • Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements

  • Communicate professionally with patients, caregivers, supervising therapists, and office staff

  • Provide one-on-one therapy services in patients' homes throughout assigned service areas

Job Qualifications
  • Active New York State PTA or OTA license/certification, as applicable

  • Basic computer proficiency for electronic documentation

  • Strong interpersonal and communication skills

  • Ability to work independently while following the established plan of care and clinical supervision requirements

  • Valid driver's license and reliable transportation for home visits

  • Home health, outpatient, rehabilitation, or skilled nursing experience preferred

  • Minimum of 1 year of clinical experience preferred
