Job description

Empath Home Health, a member of Empath Health is seeking a caring full-time Home Health LPN to work with us to deliver individualized care in North Sarasota area.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.
Locations: Home health visits in North Sarasota county including: North Sarasota, Longboat Key, University Parkway, Downtown Sarasota, Fruitville, & and surrounding areas.
Schedule: M-F 40 hours; weekend and holiday rotations required.
What you'll Do
  • Provides nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient's plan of care under the direction and supervision of a Registered Nurse.
  • Observes patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
  • Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
  • Performs patient and family instruction on diet, medication, and treatment.
  • Collaborate with the interdisciplinary team to ensure seamless, holistic care

  • Active Florida LPN license
  • Applicants must be a graduate of an approved nursing program
  • Requires 1-2 years of LPN experience
  • Home health care experience preferred.
  • Intravenous (IV) Certification strongly preferred.
