Job description

Summary/Objective
A summary of the Intake Coordinator is conducting thorough intake interviews and prescreens to gather
vital information to assess and determine eligibility for VTC's programs. The objective of the Intake
Coordinator is to facilitate access to necessary services and support, contributing to the overall well-being
and stability of Veterans experiencing homelessness. The Intake Coordinator will have a full understanding
of the Veterans statuses and their homes, providing reports and debriefs to case managers and the
program director.
Essential Functions
Intake and Discharges
• Explains general resident policies, rules, and fees during the intake process
• Unit placements for clients and bed count management
• HMIS Entry
• Assigns new intake to case managers with Program Director collaboration
• Assist clients with ordering DD-214's upon intake or when appropriate
• Code bedroom and front door of Veteran residence
• Manage the GPD and HCHV intakes/beds in collaboration with the Program Director
• Collaborate with all GPD case managers regarding clients and supports for new intakes
• Arrange intake meetings with case management
• HMIS exits
• Assist Case Managers and Marina PD with Safety Discharges
Administrative/Clerical
• Reporting bed census to VA
• Outreach
o Veterans Hall in Santa Cruz
o Utilizing By Name List
o Regularly contacting outside providers for potential referrals
• Prescreen potential residents for eligibility and suitability for Grant Per Diem (GPD) or Healthcare
for Homeless Veterans program (HCHV)
• Maintain knowledge of Veteran's status when choosing placements
• Manage resident database
• Ordering and shopping for Veteran household supplies
• Create and maintain a list of resources for Veterans who are not eligible for VTC's programs. Provide
resources and referrals to walk-ins at Martinez Hall
• Maintain confidential files of past clients
• Manage Air Table
• CARS assessments
• Create data reports
• Maintain CHSP data quality standards
External
• Community partnership engagement: CAT meetings with community providers to discuss homeless
cases, manage the Veterans By Name List in collaboration with Nations Finest and VA
• Participate in community events on behalf of VTC
• Interact with government and outside agencies
• Collaborate with VA liaisons on all matters affecting GPD and HCHV
• Main point of contact for Marina PD and other Law Enforcement Agencies in the area
Other
• Provide weekend support to case management if necessary
• De-escalation of clients at the VTC and on Hayes
• Weekly unit checks, at minimum twice a week, to ensure community safety and compliance (i.e.,
removing any drugs, weapons, guests, etc.)
• Maintain a case load of up to six clients, if necessary
Supervisory Responsibility
This position has no supervisory responsibilities.
Work Environment
While performing the duties of this position, the employee travels by automobile and is exposed to
changing weather conditions. The employee works in varied residential environments based on assigned
caseload. This position performs tasks that may involve exposure to blood, body fluids or tissues. All
appropriate and proper safeguards, procedures, and policies are expected to be followed at all times.
Physical Demands
The physical demands described here are representative of those that must be met by an employee to
successfully perform the essential functions of this job. While performing the duties of this job, the
employee is regularly required to talk or hear. The employee is regularly required to stand, walk, sit; use
hands to finger, handle or feel objects, tools, or controls; and reach with hands and arms. The employee
frequently lifts and moves up to 30 pounds. Specific vision abilities required by this job include close vision
and the ability to adjust focus.
Position Type/Expected Hours of Work
This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 4:00 p.m.
Occasional evening and weekend work may be required as job duties demand.
Travel
Travel is primarily local during the business day, although some out-of-area may be expected.
Required Education and Experience
Must have and maintain a valid state driver's license, maintain automobile insurance coverage, and have
access to an automobile.
Preferred Education and Experience
Experience working with the homeless population and/or Veterans with a bachelor's degree in social work,
psychology or related field
Additional Eligibility Qualifications
• Ability to read, analyze, and interpret human service periodicals, professional journals, technical
procedures, and governmental regulations. Ability to write reports, business correspondence and
procedure manuals. Ability to effectively present information and respond to questions from groups
of managers, clients, customers, and the public.
• Ability to define problems, collect data, establish facts and draw valid conclusions.
