Description
The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y.
Jordan Health is now seeking an Optometrist who, under the guidance of the Chief Medical Officer, is primarily responsible for providing comprehensive vision care to patients so they can care for their eyes and access tools to support their eyesight. Duties include conducting eye inspections, identifying signs of disease or injury and prescribing corrective lenses.

Requirements
The Optometrist will ensure that:
• Eye exams include History, Visual Acuities, Bio microscopy, Retinoscopy, Refraction, Tonometry, and Ophthalmoscopy.
• Writing copy of prescriptions. Writing prescriptions consistent with level 1 and/or 2 therapeutics in addition to basic license.
• Writing prescriptions for lenses.
• Writing referrals.
• Maintain client confidence and protects operation by keeping information confidential.
• Contributes to team effort by accomplishing related results as needed.
• Reliable attendance in order to be present to maintain effective relationships and keep up with position responsibilities.
Education And Experience Required:
• Optometry License and use of diagnostic medications and level one therapeutics.
