Key Responsibilities
  • Conduct comprehensive assessments including biopsychosocial evaluations to inform treatment decisions and tailor care to individual patient needs.
  • Develop, implement, and adjust personalized treatment plans using evidence-based interventions, with a focus on emotional support and coping strategies for patients and families.
  • Maintain accurate clinical records, ensuring compliance with confidentiality protocols and legal regulations while utilizing digital record-keeping systems.
  • Collaborate closely with a multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals, to ensure a holistic approach to patient care.
  • Engage in client education and provide guidance for patients and their families to navigate the emotional and psychological aspects of hospice care.

Patient Population / Client Focus

Qualifications
  • A Master's degree in Social Work from an accredited institution is required.
  • A valid LCSW license in the state of Illinois.
  • At least 2 years of clinical experience, preferably in a mental health or hospice setting.
  • Demonstrated proficiency in solution-based psychotherapy interventions and trauma-informed care.
