Position Summary
Seeking a compassionate Registered Nurse (RN) to provide skilled hospice care to patients and families in home and community settings. The RN evaluates patients for hospice eligibility, develops and implements individualized care plans, and collaborates with the interdisciplinary team to ensure quality end-of-life care. This position requires strong assessment, communication, documentation, and customer service skills while maintaining patient confidentiality and dignity.
Responsibilities
- Assess patients for hospice eligibility and ongoing care needs.
- Provide skilled nursing care according to physician orders and care plans.
- Monitor patient condition and communicate changes to the care team.
- Educate patients, families, and caregivers regarding disease progression, symptom management, and hospice services.
- Maintain accurate, timely clinical documentation.
- Participate in interdisciplinary care planning and team meetings.
- Support patient rights, cultural diversity, and individualized care.
- Provide backup on-call coverage as assigned.
- Travel throughout the service area to perform patient visits.
Qualifications
- Graduate of an accredited School of Nursing; BSN preferred.
- Current Georgia RN license required.
- Minimum one year of direct patient care experience in hospice, oncology, medical-surgical, or home health.
- Hospice or Oncology certification preferred.
- Current CPR certification required.
- Valid driver's license, reliable transportation, and automobile insurance required.
- Proficiency in Microsoft Word, Outlook, and electronic documentation systems.
- Strong clinical assessment, communication, and customer service skills.
Work Environment
Full-time position working 30-40 hours per week with rotating days, evenings, weekends, and occasional holidays as needed. Homecare RNs use their personal vehicle for patient visits and may travel more than 500 miles per week.
