Your Role

The Access Navigator plays a critical role in supporting the coordination and administrative operations of recovery, transition, and community-based services. This position ensures the timely completion of essential documentation, facilitates communication across multidisciplinary teams, and assists with the coordination of care to support individuals transitioning to appropriate levels of community living and behavioral health services.

Core Responsibilities

  • Scan, upload, and organize client documents, and complete declination of letters, discharges, and related tracking within the health record
  • Obtain medical records and collect required signatures, releases of information, and other missing documentation (e.g., IATP)
  • Support clients through admission, intake, and consent processes with a welcoming, compassionate approach
  • Help coordinate handoffs between programs and services to support continuity of care
  • Collect and track data for grants, compliance, and quality improvement measures, and manage program-specific reports to support quality care, administrative workflows, and performance outcomes
  • Support leadership in tracking key staff goals and outcomes, including productivity tracking and related reporting processes
  • Schedule staffings, follow-up meetings, and team schedules, and support staff coverage needs and day-to-day adjustments
  • Attend team meetings, record minutes/notes, and distribute follow-up action items to attendees
  • Maintain training records and certifications in designated systems
  • Assist with program logistics, materials, and general office operations
  • Take on additional administrative or client-facing tasks depending on program and department needs

Knowledge, Skills, and Abilities

  • Strong organizational and time-management skills with the ability to manage multiple tasks, deadlines, and competing priorities
  • Skilled in document management, data entry, scheduling, and electronic clerical workflows
  • Competent in the use of electronic health record systems, digital signature platforms (e.g., DocuSign), and Microsoft Office/Google applications
  • Strong written communication skills including note-taking, documentation, and professional correspondence
  • Effective verbal communication skills with staff, clients, external partners, and multidisciplinary teams
  • Skilled in problem-solving, follow-through, and navigating barriers to care
  • Customer service skills with a focus on service delivery and professional interactions

Qualifications

  • High school diploma or equivalent
  • Minimum of 21 years of age
  • A valid driver's license, reliable means of transportation, and proof of automobile insurance
