Work Environment

This role is full-time and primarily onsite, with a focus on delivering home care services. You will operate in a collaborative environment where you are supported by a multidisciplinary team. The work process includes direct interaction with patients in their homes, providing you with the unique opportunity to observe and understand their day-to-day challenges. Your average workweek will consist of 40 hours, with variable weekends as needed to accommodate patient care schedules.

Key Responsibilities
  • Examine and evaluate patients to develop comprehensive treatment plans tailored to individual functional limitations.
  • Collaborate with healthcare providers to ensure continuity of care and offer consultations regarding patient treatment plans.
  • Maintain open, respectful interactions with patients of all ages and backgrounds, adhering to our high customer service standards.
  • Conduct detailed evaluations and interpretations of findings, followed by the implementation of effective treatment strategies.
  • Continuously assess progress, modify treatment and discharge plans, and provide patient education throughout the recovery process.
  • Document all evaluations, treatments, and charges accurately in compliance with applicable laws and professional standards.
  • Participate in leadership initiatives, including community outreach, program development, and marketing efforts.
  • Supervise and mentor assigned support staff, fostering a collaborative approach to patient care.
Patient Population / Client Focus

Your role will focus on a diverse patient population, ranging from post-surgical rehabilitation to those with chronic pain or mobility issues. Your ability to connect with patients and their families will be crucial in creating a trusting environment where effective treatment can take place. You will also be expected to adapt treatment plans according to the cultural and personal preferences of patients, ensuring inclusivity in therapy.

Qualifications
  • A Doctorate in Physical Therapy (DPT) is required.
  • A valid state license in Physical Therapy is mandatory.
  • Basic Life Support (BLS) certification.
