CNA Client Care Coordinator

Assured Care at Home - a member of Assured Healthcare Staffing, is looking for a CNA Client Care Coordinator to join our nursing department. This hybrid role blends hands-on caregiving with behind-the-scenes coordination โ€” you'll make sure every client has reliable, well-matched care coverage, while occasionally stepping in to provide direct care yourself. In this role, you'll report to our Healthcare Agency Nurse Manager.

What You'll Do:

  • Schedule field staff for private-duty home care and school-based clients, matching care plans, availability, and preferences.
  • Manage daily staffing to make sure every shift is covered.
  • Build strong relationships with field staff through regular communication and recognition.
  • Keep clients and field staff updated on schedules and availability.
  • Maintain accurate, up-to-date scheduling records and documentation.
  • Step in to provide hands-on CNA and personal care when needed, including ambulation, exercises, and essential household support.
  • Report changes in a client's condition or needs to the Agency Nurse Manager.
  • Ensure that care notes and documentation daily. Participate in on-call rotation: one night per week and one weekend per month.
  • Respond to urgent client care needs as they arise.

Requirements:

  • Active State of Illinois CNA certification, in good standing
  • 2+ years of home care experience
  • Experience in scheduling, staffing, or office coordination preferred
  • Ability to lift and transfer patients as needed
  • Strong communication skills and the ability to multitask
  • Ability to stand and walk for extended periods
  • Comfortable using a computer and phone for scheduling and documentation

Pay rate: $20.50/hr with quarterly bonuses.
