Job description

This is an on-call clinical position. This position facilitates the social service triage, assessment, and care coordination needs of assigned patients of Palm Beach County Fire Rescue's (PBCFR) Mobile Integrated Healthcare (MIH) Program. The employee may also serve as a liaison between PBCFR and local social service agencies/healthcare providers with a relationship to the MIH Program. This position reports directly to the Medical Social Work Coordinator. The employee is expected to be flexible, independent, and have the ability to initiate and follow up on patient care related to assigned patients within the MIH Program.
QUALIFICATIONS
This position requires qualified applicants to possess the following:
โ€ข A Master's Degree in Social Work; minimum of two (2) years of experience in medical social work within a healthcare setting such as hospitals, home care agencies, primary or specialty care clinics, or hospice facilities.
PREFERRED QUALIFICATIONS
Preferred qualifications include (must specify on application):
โ€ข Licensed Clinical Social Worker (LCSW) with experience in medical social work in both home and acute care settings.
