On-Call Medical Face-to-Face Assessment Provider
Physician, Physician Assistant, Nurse Practitioner, or Registered Nurse
Teaching Family Homes of Upper Michigan
Position Summary
Teaching Family Homes of Upper Michigan is seeking a qualified Physician, Physician Assistant, Nurse Practitioner, or Registered Nurse to provide face-to-face assessments for youth on an as-needed, on-call basis.
This position supports agency compliance, youth safety, and appropriate post-restraint review requirements. The provider will be contacted only when a qualifying restraint occurs and a face-to-face medical assessment is required. The provider must be available to respond and complete the required assessment within one hour of the need being identified.
This is an on-call contracted position that includes a retainer fee for availability, as well as a separate face-to-face fee-for-service payment when an assessment is completed.
Essential Responsibilities
The On-Call Medical Face-to-Face Assessment Provider will be responsible for:
โ€ขRemaining available during agreed-upon on-call periods.
โ€ขResponding promptly when contacted by agency leadership or designated staff.
โ€ขCompleting a face-to-face assessment of a youth when required following a restraint.
โ€ขCompleting the assessment within one hour of the identified need.
โ€ขAssessing the youthโ€™s physical condition, safety, and any immediate medical concerns.
โ€ขDetermining whether additional medical evaluation or emergency care is needed.
โ€ขDocumenting the assessment in accordance with agency procedures and applicable requirements.
โ€ขCommunicating assessment findings to the designated agency staff.
โ€ขMaintaining professionalism, confidentiality, and youth-centered care at all times.
โ€ขCompleting required initial agency training, including training related to the agencyโ€™s utilized restraint model.
โ€ขParticipating in any required refresher training related to the role.
Minimum Qualifications
Qualified applicants must be one of the following:
โ€ขLicensed Physician
โ€ขLicensed Physician Assistant
โ€ขLicensed Nurse Practitioner
โ€ขLicensed Registered Nurse
Applicants must also:
โ€ขHold an active license in good standing in the State of Michigan.
โ€ขHave the ability to complete in-person, face-to-face assessments within the required timeframe.
โ€ขBe willing to complete agency-required training prior to serving in the on-call role.
โ€ขHave experience or comfort working with youth, behavioral health needs, trauma-informed care, or crisis situations.
โ€ขDemonstrate sound clinical judgment and the ability to remain calm in urgent situations.
โ€ขMaintain appropriate professional boundaries and confidentiality.
โ€ขPass any required background checks or credential verification processes.
Preferred Qualifications
Preference may be given to candidates with experience in:
โ€ขPediatrics or adolescent health
โ€ขBehavioral health or psychiatric care
โ€ขResidential treatment settings
โ€ขCrisis intervention
โ€ขTrauma-informed care
โ€ขYouth-serving systems, child welfare, juvenile justice, or mental health stabilization programs
Schedule and Availability
This is an as-needed, on-call position. Specific on-call expectations, coverage periods, response procedures, and availability requirements will be determined through agreement with Teaching Family Homes.
The provider must be able to respond when contacted and complete the required face-to-face assessment within one hour when an assessment is needed.
Compensation
This position will be compensated through:
โ€ขA retainer fee for agreed-upon on-call availability; and
โ€ขA separate face-to-face fee-for-service payment for each completed assessment.
Specific compensation rates and payment terms will be outlined in a written agreement.

Training Requirements Before beginning on-call duties, the provider must complete required agency training, which may include but is not limited to: โ€ขOverview of Teaching Family Homes policies and procedures โ€ขConfidentiality and mandatory reporting expectations โ€ขDocumentation expectations โ€ขTrauma-informed care overview โ€ขAgency crisis response procedures โ€ขTraining related to the agencyโ€™s utilized restraint model Physical and Work Requirements The provider must be able to: โ€ขTravel to the designated agency location when contacted. โ€ขComplete an in-person assessment of a youth. โ€ขCommunicate clearly with youth and staff. โ€ขComplete required documentation in a timely manner. โ€ขRespond calmly and professionally in crisis-related situations.
