Your Core Responsibilities:

  • Monitor and coordinate the health and well-being of individuals supported, ensuring timely follow-up care.
  • Oversee and ensure compliance with all medication administration procedures, policies, and inventories.
  • Train, supervise, and evaluate direct support staff and LPNs in nursing practices and medication protocols.
  • Maintain accurate, timely medical documentation and electronic health records in compliance with regulations.
  • Collaborate with families, advocates, physicians, and team members to provide person-centered care.
  • Respond to health emergencies, conduct clinical assessments, and coordinate post-discharge care.
  • Provide medical coverage across assigned sites and review RN on-call reports as needed.

What You Bring:

  • Current, unencumbered NYS Registered Nurse (RN) license.
  • At least 1 (one) year of nursing experience.
  • Strong clinical judgment, communication, and documentation skills.
  • Ability to work independently and collaboratively with interdisciplinary teams.
  • Valid NYS driver’s license and reliable transportation.
  • Experience supporting individuals with developmental disabilities preferred but not required.
