Job Posting: Registered Nurse- On-Call Triage

Location: FULL REMOTE(NEW YORK STATE RESIDENTS ONLY), no on-site availability necessary

Department: Nursing

Employment Type: Per Diem, On-Call

Schedule: Evenings (Monday 4:00pm – Saturday 8:00am)

Weekends (8:00am Saturday – 8:00am Monday)

Salary: Stipend of $40.00 per weekday evening

Stipend of $75.00 per weekend day/evening

Plus hourly rate paid for time worked/calls taken.

Hourly rate: $35.00 - $38.00

About the role:

Under general supervision, provides comprehensive health services. Works collaboratively with other providers to facilitate client wellness. Provides remote, off work hours telephone triage to staff calls from OPWDD & OASAS residential group homes. Maintains all applicable confidentiality and ethical standards of relevant governing bodies. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.


Essential Duties and Responsibilities

  • Remote position, on call overnights, weekends and holidays
  • Evaluates the comprehensive health assessment of clients.
  • Identifies immediate health care needs.
  • Makes appropriate referrals and recommendations based on outcome of triage logic
  • Collaborates with appropriate Agency staff, and related community organizations to assure coordination of client care and services if needed.
  • Completes required electronic patient record documentation in real time.
  • Patient Assessment and Triage: Respond to urgent calls, assess symptoms, and determine the appropriate care over the phone.
  • Provide staff education and care instructions within the RN scope of practice
  • Document staff education accurately and in a timely manner in the EMR.
  • Demonstrates effective communication with staff to assure appropriate understanding of treatment requirements and objectives of those actions.
  • Strong clinical assessment and critical thinking skills.
  • Excellent verbal and written communication.
  • Strong interpersonal and customer service skills.
  • Extensive knowledge of anatomy, physiology and nursing care standards
  • Analytical skills to assess patient condition
  • Effective oral communication skills to interview staff and explain next steps as well as to pass information on to other members of the care team
  • Ability to work with diverse and vulnerable patient populations.
  • Excellent organizational, computer, and documentation skills.
  • Possess and maintain accurate knowledge of all regulatory and practice regulations and treatment protocols with special attention to HIPAA requirements
  • Always demonstrates the highest standards for ethical and professional conduct.
  • Proficiency with computer-based applications to independently complete work and add/use data
  • Demonstrate a thorough knowledge of services provided both by the Agency and by external service providers and maintain contacts as needed to efficiently access care for clients.
  • Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

Requirements:

Qualifications

Education: Minimum of an Associate in Applied Science degree in Nursing required. Bachelor of Science Degree is preferred.

Credentials: NYS Professional Registered Nursing License in good standing.

Experience: At least two years of experience related to nursing services for those with Intellectual Developmental Disabilities.
