At Lifted Hospice, our On-Call RN Case Managers play a vital role—combining clinical excellence with genuine compassion to provide care that honors life and brings comfort when it matters most.


As an On-Call RN Case Manager, you’ll deliver personalized nursing care in the patient’s home or care setting. You’ll collaborate closely with the interdisciplinary team to build and follow care plans that reflect the patient’s goals, support their family, and meet the highest standards of clinical, ethical, and emotional care. You’ll also educate and equip families to help them understand what to expect, how to provide comfort, and how to feel confident in their role during a sacred season of life.


Your Role in the Mission

As an On-Call Registered Nurse (RN) Case Manager at Lifted Hospice, you’ll provide expert care and steady support for patients and families facing end-of-life. You’ll lead the bedside experience—managing symptoms, guiding care, and helping families feel confident and supported.


Key Responsibilities Include:

  • Assessing needs – Completing thorough assessments to guide care based on each patient’s physical, emotional, and clinical needs.
  • Managing symptoms – Delivering skilled hospice care to help manage pain and improve quality of life.
  • Working as one team – Collaborating with the full care team, the patient, and their loved ones to create and follow a unified care plan.
  • Keeping clear records – Documenting care and updates accurately in line with all standards and requirements.
  • Supporting families – Educating and encouraging caregivers, offering emotional support, and helping families feel prepared and confident.


Skills and Qualifications

  • Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program, with an active state license and clinical skills that meet current standards of nursing practice.
  • Experience as a registered nurse in a clinical care setting. Hospice experience preferred.
  • Excellent interpersonal skills.
  • Must read, write and comprehend English.
  • Current driver’s license, reliable transportation, and valid auto liability insurance.


Where You’ll Work

In this role, you’ll spend most of your time providing care wherever patients call home. Whether that’s a private residence, memory care facility, or other long-term care facility.


You will also meet with your local team of social workers, chaplains, physicians, fellow nurses and our medical director to align patient care plans and ensure every patient and family receives the holistic, compassionate support they deserve bi-weekly.

Schedule

On­-Call coverage will be the following bi­-weekly until further notice:

  • Monday through Thursday 5:00 p.m. to 8:30 a.m. AND Friday 5:00 p.m. to Monday 8:30 a.m.
