Job description

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.


The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.


Resource RN

The Resource RN is a float position that is responsible for providing skilled nursing intervention and supportive care in patients' homes, nursing homes or long term care facilities. This RN administers medications and treatments as prescribed by the physician and plan of care. This position works 2(8 hour) shifts and 2(12 hour) shifts rotating weekends and holidays.


Key Responsibilities:

  • Conducts comprehensive patient assessments and develops, updates, and manages individualized care plans for hospice patients
  • Provides skilled nursing care, including medication administration, treatments, and patient/family education
  • Collaborates with the interdisciplinary team and participates in IDT meetings to ensure coordinated patient care
  • Supports patient needs through on-call coverage, crisis response, and assistance with daily living activities
  • Ensures compliance with regulatory standards, maintains accurate documentation, and meets quality and performance metrics
  • Requirements include RN with a minimum of two (2) years nursing experience, at least one of which is in the area of public health, home care or hospice nursing preferred. Incumbent must be self directed and able to work with minimal supervision.
  • Must be able to work 40 hours a week with a minimum of 20 hours on call/month, may increase based on staffing


Education/Experience:

  • Current RN license
  • Minimum of 2 years' experience, at least 1 year area of public health, home care, or hospice nursing is preferred
  • Valid driver's license and insured, reliable vehicle
