Licensed Practical Nurse (LPN)
Location: Oil City, Pennsylvania, United States

About the Role:

As a valued member of the Oil City Nursing and Rehab team, we are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our family of caregivers. Located just outside the hustle and bustle of town in a peaceful, comfortable setting, our facility is dedicated to providing personalized customer experience and supportive care to our residents.

Responsibilities:

  • Provide direct patient care to our residents, including but not limited to vital signs, medication administration, and wound care as ordered by the attending physician

  • Assist with ADLs (Activities of Daily Living) such as bathing, dressing, and grooming

  • Develop and implement individualized care plans, in collaboration with the multidisciplinary team

  • Monitor and document resident progress, reporting any changes or concerns to the nursing staff

  • Work closely with the LPN/LVN team to ensure consistent care and communication

  • Participate in quality assurance activities to maintain high standards of care

Requirements:

  • Current licensure as an LPN/LVN in the state of Pennsylvania

  • High school diploma or equivalent required

  • Basic Life Support (BLS) certification required

  • Ability to work effectively in a fast-paced environment with a focus on patient-centered care
