Description
RN Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities
Requirements
Essential Job Duties & Responsibilities
  • Manage and coordinate care for an assigned hospice patient caseload.
  • Complete comprehensive nursing assessments and ongoing patient evaluations.
  • Develop, implement, and update individualized Plans of Care.
  • Perform routine nursing visits, admissions, recertifications, and discharge visits as assigned.
  • Coordinate services with physicians, nurse practitioners, social workers, chaplains, CNAs, volunteers, and other members of the interdisciplinary team.
  • Monitor patient symptoms and provide appropriate interventions for pain and symptom management.
  • Educate patients, families, and caregivers regarding disease progression, medications, equipment, and end-of-life care.
  • Ensure all required documentation is completed accurately and timely within the electronic medical record.
  • Participate in Interdisciplinary Group (IDG) meetings and contribute to patient care planning.
  • Coordinate medication, supply, and DME needs to ensure continuity of care.
  • Respond to changes in patient condition and communicate concerns to the appropriate team members.
  • Complete HOPE assessments, supervisory visits, and other regulatory requirements as assigned.
  • Collaborate with office staff to maintain visit frequencies and regulatory compliance.
  • Participate in on-call rotation as assigned.
  • Maintain patient confidentiality and comply with all HIPAA regulations.
  • Promote positive relationships with referral sources, facilities, patients, and families.
Qualifications
  • Current Registered Nurse (RN) license in the State of Texas.
  • Minimum (2) years nursing experience required.
  • Hospice, home health, oncology, geriatrics, or case management experience preferred.
  • Current CPR certification.
  • Valid Texas driver's license, reliable transportation, and automobile insurance.
  • Ability to travel throughout the service area.
  • Strong assessment, organizational, and communication skills.
  • Ability to work independently while maintaining collaboration with the interdisciplinary team.
  • Proficient with electronic medical records and Microsoft Office applications.
Physical Requirements
  • Ability to stand, walk, bend, kneel, and lift up to 50 pounds.
  • Ability to travel to patient homes, facilities, and hospitals throughout the service area.
  • Ability to perform nursing procedures and patient care activities in various environments.
