Job description

Responsibilities:

  • Conducts initial assessment of prospective residents, including off-site visits
  • Performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident
  • Delivers health promotion services, offers disease prevention counseling, and provides self-care education to residents
  • Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
  • Fosters communication by regularly engaging with physicians, pertinent healthcare providers, and family members
  • Hires, trains, schedules, and leads the clinical team in providing exceptional care experiences
  • Coordinates the Community's infection control program while maintaining state regulatory compliance
  • Develop and oversee the Community's medication management program
  • Contributes towardquality assurance and performance improvement initiatives
  • Complies with all rules and regulations regarding confidentiality and privacy of resident information

Qualifications:

  • Maintains current state nursing licensure.LPN required, RN preferred
  • Minimum of two (2) years nursing supervisory experience
  • Prior senior/supportive living experience strongly preferred
  • Excellent written and verbal communication skills
  • Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members
  • Demonstrates exceptional interpersonal skills and ability to serve withlove,compassion, anddignity
