Occupational Therapist (OT) – Home Health & Assisted Living | Rochester & Fairport, NY | Full-Time, Part-Time & PRN

Stern at Home Therapy is hiring Full-Time, Part-Time, and PRN Occupational Therapists (OTs) to provide one-on-one care across two established Assisted Living Facilities in Rochester and Fairport, NY, while also treating community-based home health patients throughout the surrounding area.

This hybrid role offers the consistency of facility-based patients combined with the flexibility and autonomy of home health, allowing you to build a diverse caseload while treating one patient at a time.

What You'll Do:
As an Occupational Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population across two Assisted Living Facilities and in patients' homes throughout the Rochester area.

Responsibilities include:
  • Perform occupational therapy evaluations and develop individualized plans of care.
  • Provide one-on-one occupational therapy treatments in Assisted Living Facilities and patients' homes.
  • Help patients improve independence, safety, upper-extremity function, mobility, and activities of daily living.
  • Manage individualized treatment plans with clinical autonomy.
  • Complete accurate and timely clinical documentation.
  • Communicate with patients, families, physicians, facility staff, and members of the clinical team.
  • Coordinate with our administrative team regarding scheduling, patient care, and logistics.
  • Manage your caseload across both facility and community settings.

Compensation:
Full-Time W-2: 16 patient caseload
Per-Visit Option:
  • $40–$50 per follow-up visit in the facility.
  • $45-$55 per follow-up visit in the community
  • $65 per 4-unit evaluation.

Full-Time 1099:
  • $62 per follow-up visit.
  • $75 per 4-unit evaluation.

Part-Time W-2: 10 patient caseload:
  • $40–$50 per follow-up visit in the facility.
  • $45-$55 per follow-up visit in the community.
  • $65 per 4-unit evaluation.

Part-Time 1099:
  • $62 per follow-up visit.
  • $75 per 4-unit evaluation.

PRN: 5 patient caseload:
  • $62 per follow-up visit.
  • $75 per 4-unit evaluation.
