As a nurse with our organization, you will report directly to the Clinical Manager and deliver tailored, one-on-one care that empowers patients to regain their independence within the comfort of their own homes. Collaborating with a committed team of healthcare professionals, you will oversee care plans that facilitate recovery and assist patients in returning to their cherished lifestyles.
Key Responsibilities as a Home Health Registered Nurse:
- Administer skilled nursing care to patients in their residences, which includes medication administration, wound care, and patient assessments.
- Collaborate with physicians and other healthcare providers to create and implement personalized care plans.
- Monitor and evaluate patients' conditions, reporting any changes as necessary.
- Educate patients and their families on managing diseases, medication regimens, and treatment options.
- Maintain precise documentation of patient care and coordinate with other healthcare professionals.
- Communicate patient progress and care updates to the physician and Clinical Manager.
- Supervise Home Health Aides and ensure proper completion of their documentation.
Utilize Your Skills to Make a DifferenceRequired Qualifications:
- Experience in medical-surgical, ICU, ER, or acute care settings.
- Current and valid Registered Nurse license.
- Up-to-date CPR certification.
- Experience working collaboratively within a healthcare team.
- Possession of a valid driver's license, auto insurance, and reliable transportation.
Preferred Qualifications:
- At least one year of nursing experience.
- Experience in Home Health care.
Compensation and Additional Information
Pay Per Visit/Unit Rate: $44.55 - $61.28
Scheduled Weekly Hours: 40
Salary Range: The compensation range provided reflects a good faith estimate for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.
Annual Salary Range: $69,500 - $95,600
