Job Summary: The Registered Nurse Case Manager provides comprehensive case management and referral/utilization management services, coordinating patient care across the continuum to promote quality, cost-effective outcomes. The RN serves as a patient advocate and multidisciplinary team member, managing complex and high-risk cases while supporting patients, families, and healthcare providers in accessing appropriate care and resources.
Required Qualifications:

  • Education:
    • Associate Degree in Nursing.
    • Graduate from a college or university accredited by the National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE).
  • Certifications:
    • Basic Life Support (BLS) and
    • Possess one of the following certifications:
      • Commission for Case Manager Certification Certified Case Manager (CCM).
      • Certification of Disability Management Specialists Commission Certified Disability Management Specialist (CDMS).
      • Association of Rehabilitation Nurses Certified Rehabilitation Registered Nurse (CRRN).
      • American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S).
      • National Board for Certification in Continuity of Care Advanced Certification in Continuity of Care (ACCC).
      • Commission on Rehabilitation Counselor Certification Certified Rehabilitation Counselor (CRC).
      • American Nurses Credentialing Center Nurse Case Manager (RN-NCM).
      • National Academy of Certified Care Managers Care Manager Certified (CMC).
  • License: Current, full, active, and unrestricted license to practice as a Registered Nurse
  • Experience:
    • Experience in Referral Management/Utilization Management (RM/UM).
    • Two to five years of current clinical subspecialty experience in Behavioral Health.
    • Knowledge of sources for patient care data, including medical records through ESSENTRIS and CHCS, 24-hour nursing reports, logs, consults, computerized data, and RMS.
    • Experience assessing adult and geriatric patients, as appropriate.
    • Experience coordinating patient care and services as part of a multidisciplinary healthcare team.

Key Responsibilities:
  • Coordinate exceptional patient care across the healthcare continuum, serving as a trusted advocate for patients and families.
  • Manage complex cases involving high-risk, chronic, behavioral health, TBI, substance-related, catastrophic injury, and end-of-life needs.
  • Assess patient needs and barriers and develop personalized care plans that improve outcomes and access to services.
  • Coordinate referrals, appointments, resources, and follow-up care with patients, families, providers, and multidisciplinary teams.
  • Support active-duty service members, families, and retirees through effective case and utilization management.
  • Promote quality, patient-centered, and cost-effective care while identifying opportunities to improve healthcare delivery.
  • Educate and empower patients and families with resources, self-care guidance, and support navigating the healthcare system.
  • Maintain accurate clinical documentation and use electronic medical records, databases, and other healthcare technology effectively.
  • Collaborate across teams and provide consultation, education, and support to healthcare professionals and leadership.
  • Maintain compliance and clinical excellence through adherence to nursing standards, MTF policies, Joint Commission guidelines, safety requirements, and professional competencies.

Work Environment & Schedule: On site, at Walter Reed Monday to Friday, 0630 to 1500.
Physical requirements: Ability to sit, stand, walk, bend, reach, and move throughout the medical facility as needed to perform clinical and administrative duties. Ability to use computers and electronic medical records for extended periods and safely assist with patient care, equipment, and supplies as required.
Compensation & Benefits: $40.00 to $50.00 per hour + H&W
