Job description

We are seeking a Full-Time Case Manager for our Residential Treatment Facility - Adults. To ensure that all individuals receive the services they need to live, learn, work, and socialize in the community. Identify, provide, and maintain regular support for clients to ensure continuity of care. Provide a continuous flow of services, in compliance with contract and Agency regulations. Receives direct supervision from the Program Director or their designees.
Other duties, but not all, are inclusive:
  • Ensure the delivery, flow, design, and availability of appropriate services for clients, including the referral process. Scheduling evaluators and ensuring programs comply with regulatory requirements. Also, ensuring admission paperwork is complete.
  • Participate in the development and refinement of individual rehabilitation plans incorporating social, educational, vocational, self-care, and residential skills and objectives.
  • Assess service delivery gaps and recommend alternative action plans.

Qualifications:
  • B.A/B.S. degree in Human Services/Social Science, Healthcare, or a related field and 3 years of direct care experience; or any combination of experience, education, or training that would provide the level of knowledge, skills, and ability required.
  • Very good communication skills.
  • Good organizational skills โ€ข Ability to work independently and as part of a focused team.
  • Must be available for local travel and possess a valid driver's license with an acceptable driving record.

Scope:
  • Frequent talking and listening
  • Requires the ability to lift, carry, fold, kneel, reach, stack, stoop, and bend, which may include direct intervention with clients.
  • May require crisis or emergency intervention assistance.
  • Scheduling flexibility and the possibility of on-call work are necessary.
  • May be required to report to work during emergencies, including inclement weather.
  • Possess manual dexterity and fine motor skills.
