Requirements :
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience
- CHPN certification (Preferred)
Roles & Responsibilities :
- Assess and respond to the needs of patients and families in home health or hospice settings.
- Develop, document, and evaluate individualized Plans of Care (POC) to ensure quality and continuity of care.
- Educate patients and families on care, medications, and self-care practices tailored to their specific needs.
- Supervise and provide clinical direction to home health aides and nursing assistants.
