Description

American Traveler is hiring a Home Health RN with HCHB and OASIS charting experience for a field position requiring on-call rotation and same-day patient visits.

Details
  • Field-based Home Health RN role providing in-home patient care within a designated service territory
  • Day shift schedule, Monday through Friday, 8:00 AM to 5:00 PM
  • Average of 4-6 patients per day, with a minimum requirement of 30 points per week
  • Patient population includes adult and geriatric home care cases
  • Responsible for interdisciplinary care coordination and plan of care development and oversight
  • Charting must be completed using HCHB and OASIS while at the patient's home
  • Weekend coverage required as needed
  • Participation in on-call rotation required, including at least one weeknight per week
  • Must be willing to travel up to 50 miles outside the home base office
Requirements
  • Active TX or compact RN license required, must be in hand at time of consideration
  • Minimum 1 year of Home Health nursing experience required
  • Minimum 2 years of overall clinical nursing experience required
  • BLS certification required
  • HCHB and OASIS charting experience required
  • Valid driver's license required
  • Must have reliable transportation and insurance
  • Non-smokers preferred due to patient sensitivity to smoke odor
Additional Information
  • Responsible for administering skilled nursing care to patients from diverse populations across a variety of home environments
  • Coordinates care with patients, families, certifying providers, and interdisciplinary team members
  • Performs comprehensive patient assessments and collaborates on plan of care development
  • On-call responsibilities include taking after-hours calls and completing related charting
  • Dress code consists of scrubs in any color
