Always Best Care Senior Services is a Medicare-certified Home Health agency proudly serving communities across Oahu. Our mission is to help seniors remain safe, independent, and comfortable in their own homes by providing exceptional home health services focused on improving mobility, reducing fall risk, and enhancing quality of life.

We are currently seeking a compassionate, motivated, and licensed Physical Therapist (PT) to join our growing clinical team. If you're passionate about making a meaningful impact while enjoying flexibility and autonomy, we'd love to hear from you.

Compensation

  • $80–$85 per patient visit
  • $200 per Start of Care (SOC) visit
  • Full-Time Position
  • Flexible scheduling and the opportunity to build your own caseload


What You'll Do

  • Provide skilled physical therapy services in patients' homes.
  • Perform comprehensive initial evaluations, reassessments, and fall risk assessments.
  • Develop individualized treatment plans to improve strength, mobility, balance, and functional independence.
  • Educate patients and caregivers on home exercise programs and injury prevention.
  • Collaborate with nurses, physicians, and the interdisciplinary care team to ensure quality patient outcomes.
  • Complete accurate, timely, and compliant clinical documentation.


Qualifications

  • Current Hawaii Physical Therapist (PT) license in good standing.
  • Minimum of one year of clinical experience preferred.
  • Home Health experience is a plus, but not required.
  • Strong clinical judgment, communication, and documentation skills.
  • Valid driver's license, reliable vehicle, and current auto insurance.
  • Ability to travel throughout Oahu to provide in-home patient care.
  • Passion for serving seniors with compassion and professionalism.
