Position Summary:
We have an exciting opportunity to join our team as a Occupational Therapist Per Visit, Home Health Care / Eastern Long Island.
In this role, the successful candidate Provides safe, therapeutic, age appropriate occupational therapy toHelp patients restore or improve functional physical abilities, promote cognitive and behavioral changes, adapt surroundings to achieve optimal level of daily life (ADL/IADL) tasks. Uses clinical expertise to plan delivery of care through examination, planning, implementation and evaluation.

Job Responsibilities:

  • Performs other duties as assigned
  • Evaluates and assesses patients including the use of appropriate tests and measures; patient safety issues and home environment factors. Establishes plan of care with measurable and functional short termlong term goals.
  • Implements and modifies appropriate interventionstreatments based on the patient s functional status, behavioral changes, home environment, patient response and status in conjunction with the plan of care.
  • Interacts and collaborates with other members of the healthcare team to optimize patient outcomes and health care goals.
  • Educates patient and family regarding patient's functional status and plan of care. Provides home exercise program and instruct on home safety factors.
  • Re-assesses patient progress toward short termlong term goals and modifies the plan of care accordingly.
  • Completes and maintains accurate documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education, intervention and evaluation according to Federal, Department of Health (DOH) and The Joint Commission (TJC) requirements.
  • Assists leadership with achieving corporate goals, improving quality of patient care outcomes and patient satisfaction. Participates in Agency quality assessmentperformance improvement initiatives.

Minimum Qualifications:
To qualify you must have a Bachelor's Degree Occupational Therapy (Accreditation Council of Occupational Therapy Education (ACOTE) approved NYS OT program). Current and valid NYS Occupational Therapy license and registration. Basic Life Support Certification. 1- 2 years Hospital (Inpatient) or Orthopedic or Outpatient or Pediatric Rehabilitation setting. Communication (both written and oral), customer service, and critical thinking skills. Basic computer skills..
Preferred Qualifications:
1- 2 years Home Care Experience.
