Job description

Job Purpose: Provide medical nutrition therapy and diabetes education to improve outcomes and quality of life.

Essential Duties and Responsibilities

  • Provides individual medical nutrition therapy and individual and group diabetes education to meet patient goals, through in person and/or telehealth sessions
  • Assesses, evaluates, interprets, monitors, and documents nutritional status and needs of patients; uses established standards of care and evidence-based practice guidelines
  • Counsels and provides tailored nutrition education to patients and their families
  • Uses age-specific, disease-specific and culturally competent educational materials and counseling techniques to meet patient needs
  • Documents in electronic medical record to monitor patient progress
  • Communicates and collaborates with patient’s primary care provider, as well as other specialty services to coordinate care
  • Attend all mandatory trainings and department meetings, readily bringing own ideas and thoughts into the discussion
  • Develops, reviews, updates, and implements educational materials to meet the needs of patients and providers
  • Provide in-service training or presentations for other company providers and specialists, as deemed necessary
  • Contribute blog articles and recipes to company website
  • Contribute case studies and data on patient progress as part of outcomes collection and research activities
  • Continually study emerging trends and new research related to the practice of dietetics and facilitate discussion and findings with the nutrition team
  • Train patients on continuing glucose monitoring devices upon completion of training program
  • Maintain license through NC Board of Nutrition and Dietetics and credentials with the Commission on Dietetic Registration
  • Complete at least 15 diabetes-related continuing education credits annually to maintain instructor status in Avance Care accredited diabetes program, in addition to completing continuing education to maintain RDN credentials

Qualifications

Experience, Education and Licensure:

  • Registered Dietitian through the Commission on Dietetic Registration
  • Licensed Dietitian through the North Carolina Board of Nutrition and Dietetics
  • Master’s Degree, preferred
  • At least 1 year of clinical nutrition experience, highly preferred
  • Diabetes education experience, preferred

Knowledge, Skills, and Abilities:

  • Strong written and verbal communication
  • Strong computer skills
  • Strong attention to detail
  • Team player
  • Efficient time management and workload prioritization

Language Skills:

Ability to communicate well in writing and in person. Ability to read and interpret common medical terminology.
