Primary Registered Nurse (RN)

About the Role:

As a Primary Registered Nurse, you will serve as the lead case manager for patients in both home and facility settings. You'll partner closely with float nurses and an interdisciplinary team to deliver consistent, high-quality, patient-centered care.

This role is ideal for a nurse who values autonomy, teamwork, and meaningful patient relationships. When caseloads fluctuate, you'll step in to support your team, ensuring every patient receives seamless care.


Territory: Green Bay area (with occasional travel outside the territory)


 ***During periods when your caseload is lighter, you will be expected to support your team by assisting with patient visits and transitions, serving as a Float Registered Nurse as needed. This collaborative approach ensures that all patients receive the highest level of care, regardless of fluctuating caseloads. This role is in the Green Bay territory and may travel outside of their territory at times.*** 


 Schedule:  Four 9-hour shifts, 4 days per week. Hours will be either 7 a.m.-4 p.m. or 8 a.m.-5 p.m., depending on POD needs. Minimum every 3rd weekend (currently working every 8th weekend - 12-hour shifts) on call 10-15 shifts per year, 2-3 holidays per year  


What You'll Do:

  • Provide compassionate hospice care and case management
  • Assess patient and family needs and adjust care plans accordingly
  • Respond to urgent situations and initiate crisis care when needed
  • Collaborate with physicians and interdisciplinary team members
  • Educate and support families through the end-of-life journey
  • Administer medications and document care in the EMR
  • Delegate and guide LPNs and CNAs
  • Manage daily priorities and assist with admissions

Qualifications

  1. Current unrestricted Registered Nurse licensure from the State of Wisconsin
  2. Minimum of 2 years RN experience required, equivalent nursing experience may be considered
  3. Valid Wisconsin Driver's License, reliable transportation and proof of current automobile insurance coverage
