Be the Comfort That Changes Everything

Some healthcare roles keep you behind a desk or rushed between rooms. This isn't one of them. At Transitions Hospice, our CNAs show up in the place that matters most โ€” a patient's home โ€” and provide the kind of unhurried, dignified care that makes a real difference.

We've reimagined what hospice looks like: 24/7 support that addresses the physical, emotional, and spiritual needs of patients and their families.

If you're a CNA who leads with heart and wants to feel the weight of your work in the best possible way, we'd love to meet you.

What You'll Do

As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team โ€” not just a support role, but a trusted presence for patients and families. Your responsibilities include:

  • Providing personal care with dignity, comfort, and respect at the center of everything
  • Attending to patient needs and requests promptly and compassionately
  • Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed
  • Observing patients and reporting changes to the care team accurately and in a timely manner
  • Communicating with patients and families in a positive, ethical, and confidential manner
  • Maintaining thorough documentation in line with organizational standards
  • Upholding the hospice philosophy with professionalism and genuine compassion

Benefits We Offer

  • Competitive pay
  • Medical / Dental / Vision Insurance plans
  • Long & Short-Term Disability
  • 401(k) with company match
  • Auto Allowance & Gas Card
  • Company provided equipment
  • Paid Time Off

Why Choose Transitions Hospice?

At Transitions, CNAs aren't an afterthought โ€” they're the heartbeat of the care we provide. We give you the support, tools, and team to show up fully for your patients, every single day. Our people truly are the difference, and we invest in them accordingly.
