Job description

TEMPORARY FULL-TIME POSITION UNTIL MARCH 2027

Reporting to the Manager, Clinical Services the Nurse Care Coordinator will, under general supervision, provide intake and assessments as well as direct client care and related support, including client and health education and counseling for individuals requiring access to primary care or other programing as referred by affiliated providers, Windsor and Essex County schools and other community locations as appropriate. Responsibilities will also include consulting and liaising with community care providers/hospitals members and cross-functional partners who have roles supporting the client journey to assist in providing integrated care.

RESPONSIBILITIES & DUTIES:

  • Complete intake and assessments, including but not limited, to collecting medical, social, psycho-social, and family history.
  • Develop and navigate care plans with clients to help identify needs and goals.
  • Communicate and consult with interdisciplinary team members as well as external community partners as needed.
  • Participate in case conferencing to discuss best practices and adjust care plans.
  • Provide client education through discussion, workshops, and wellness checks, leveraging printed materials, research on best-practices and connections to community resources.
  • Facilitate, pursue and track referrals to in-house services as well as external community partners.
  • Monitor ordering, delivery and expiration dates of medications.
  • Maintain clinic schedules.
  • Triaging clients upon arrival to clinic.
  • Perform vital signs, dressings, suture and staple removal, urine dips, throat swabs, ear syringing and assist with medical examinations.
  • Immunization administration to clients of all ages (IM, SC).
  • Follow Medical Directives
    Adhering to standards as set out by the College of Nurses of Ontario and agency Policy and Procedures.
    Other duties as assigned, and which are within the scope of the position and which further the objectives/goals of the organization.
  • Will provide services at various CMHA and community partner locations and attend community events within Windsor and Essex County as required.


EDUCATION & SKILLS:

  • Requires knowledge and skills acquired through completion of a Registered Practical Nurse Diploma from a recognized post-secondary institution. Current registration with the College of Nurses of Ontario as a Registered Practical Nurse, in good standing is required. Certificate in Mental Health Nursing is a preferred asset.
  • Experience supporting clients with mental health and addictions and chronic and complex needs. Experience with motivational interviewing skills as it relates to completing intakes to elicit and identify client need. Experience performing standard clinical procedures according to established protocols.
  • Ability to work independently and collaboratively with members of the health care team. Knowledge of mental health & relevant legislation preferred.
  • Thorough knowledge of nursing principles, procedures and practices in accordance with the Standards of Practice of the College of Nurses of Ontario and the regulations of the Regulated Health Professions Act (RHPA). Familiarity and experience with implementing screening tools used in mental health and addiction. Knowledge of client evaluation and triage procedures and ability to triage and aid in patient flow. Knowledge of client care charts, client histories, and experience documenting in an Electronic Medical Record. Knowledge of inventory management practices. Knowledge of socio-economic and cross-cultural issues. Comfortable and competent with people who are exploring their gender.
  • Computer literacy skills and working knowledge or various Microsoft Windows application is required.
  • Bilingualism in both official languages at the advanced level is considered an asset.
  • A valid Ontario Driver's License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.


Hours of Work: Thirty-Five (35) hour work week. The agency's regular hours of operation are Monday to Friday, 8:30 AM to 4:30 PM. Flexible hours are required to meet client and service needs.

Salary range: Grade 8, $35.71 - $42.25 per hour

Per 2022 - 2026 Collective Agreement


Applicable shift premiums are paid in accordance with the Collective Agreement, Article 17.11.
