JOB SUMMARY
Siloam Mission's Medical Transitional Care Unit (MTCU) is a compassionate 32-bed facility supporting individuals experiencing homelessness who have medical needs that do not require hospitalization but exceed what can be safely managed in shelter or the community. The MTCU provides time-limited transitional housing integrated with medical, mental health, and social supports to improve health stability and housing outcomes.
Guided by person-centered, strengths-based, trauma-informed practice, and rooted in harm reduction and Housing First principles, the MTCU operates through a multidisciplinary team approach.
The MTCU Registered Psychiatric Nurse (RPN), Registered Nurse (RN), or Licensed Practical Nurse (LPN) with extensive mental health experience provides direct, front-line nursing care to residents with complex mental health, substance use, and physical health needs. Residents may be actively using substances, experiencing acute or chronic mental illness, and exhibiting challenging or unpredictable behaviors. This role is essential in supporting resident safety, stabilization, emotional regulation, and engagement in care, while working closely with medical, housing, and social service partners to prevent unnecessary emergency department visits and hospital admissions.
