MTK Healthcare is actively seeking an experienced Home Health Registered Nurse (RN) for a 13-week contract opportunity with VNA Hospice of the Southwest Region in Rutland, Vermont.

Duration: 13 Weeks (Contract)
Shift: Sunday - Thursday | 8:00 AM - 4:00 PM (On-call required)
EMR System: McKesson
Pay Rate: Up to $72.00 per hour
Role Overview
The Home Health RN will deliver patient-centered care using the nursing process, acting as a case manager to coordinate and manage care plans. This role requires strong experience in OASIS documentation, home care, and interdisciplinary collaboration to ensure high-quality patient outcomes.
Key Responsibilities

  • Conduct patient assessments and develop individualized care plans
  • Complete and manage OASIS documentation accurately and timely
  • Provide skilled nursing care including IV therapy, wound care, and infection control
  • Coordinate care with physicians, therapists, and interdisciplinary teams
  • Educate patients and caregivers on treatments, medications, and care plans
  • Manage case loads and ensure compliance with productivity standards
  • Maintain accurate documentation using McKesson EMR
  • Participate in case conferences and quality improvement initiatives
Requirements
  • Active RN License (State of Vermont or eligibility upon acceptance)
  • OASIS experience required (Mandatory)
  • Minimum 2 years of Home Health or relevant nursing experience
  • BLS Certification required
  • Strong case management and documentation skills
  • Experience with high-tech skills (IV therapy, wound VAC, complex wound care) preferred
