Carease Health is seeking an experienced Nurse Practitioner (NP) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.

Requirements:

  • Active NP or PA license (in the state of practice)

  • 1+ year of experience in:
