The Role:
We are currently looking for a Registered Nurse (RN) Case Manager to support our patients in the Gallup area. This is a part-time position with per-visit pay at $45/hour, perfect for experienced RNs who are passionate about making a difference while maintaining flexibility.
Key Duties:

  • Perform initial and ongoing assessments to identify patient and family care needs
  • Collaborate with physicians and care teams to develop and implement individualized care plans
  • Provide skilled nursing services, including pain and symptom management
  • Coordinate care and referrals, ensuring continuity and support across services
  • Educate families on hospice care processes, comfort measures, and resources
  • Document all patient care in compliance with hospice policy and applicable regulations
  • Maintain HIPAA compliance and adhere to state/federal standards
  • Oversee support staff such as LVNs and Home Health Aides as needed
  • Attend interdisciplinary team meetings and contribute to care planning
  • Participate in scheduled visits and on-call rotation to provide 24/7 coverage
Requirements:

  • Graduate of an accredited nursing program
  • Active RN license in the state of New Mexico
  • One year of nursing experience (hospice or palliative care preferred)
  • Excellent clinical judgment and communication skills

  • Must have reliable transportation, a valid driver's license, and auto insurance
