What You'll Do
The Registered Nurse - Skilled Nursing Facility serves as a leader of the healthcare team at Novant Health within a skilled nursing facility, providing care in a post-acute, non-hospital setting with a focus on recovery, maintenance and long-term support rather than acute episodic treatment. This role cares for short-term rehabilitation residents and long-term residents with complex, chronic, and age-related conditions. The Registered Nurse - Skilled Nursing Facility establishes authentic, personalized relationships with residents and their support systems, collaborating with physicians and an interdisciplinary team-including nursing assistants, rehabilitation therapists, and social services-to assess, plan, implement, and evaluate individualized plans of care.
The Registered Nurse - Skilled Nursing Facility ensures compliance with CMS regulations and state and federal guidelines, contributes to the interdisciplinary care planning process including Minimum Data Set (MDS) assessments, and maintains survey readiness. The Registered Nurse - Skilled Nursing Facility provides clinical oversight to nursing staff while managing medication administration, treatments, and changes in resident condition.
Utilizing best scientific evidence and compassion, the Registered Nurse - Skilled Nursing Facility supports residents in achieving optimal health outcomes or a dignified end-of-life experience, including collaboration with palliative and hospice services.
