The Role

Amae Health is seeking a driven, relationship-oriented Community Liaison to own referral growth in the Los Angeles region. This is a hybrid role: part territory-building business development, part hands-on patient advocate. You'll build and manage a network of hospital systems, EDs, discharge planners, and outpatient providers to create a consistent pipeline of qualified referrals - and you'll personally walk many of those patients through the warm handoff into care, meeting them in the hospital, at home, or in the community to make sure the transition actually holds. This full-time role requires a home address within the Los Angeles region and regular travel throughout the territory.

If you've built a book of business before; in healthcare sales, referral management, or provider partnerships, and you also care enough about the outcome to stay with a patient through a hard transition, this is your territory to build.

What You Will Own

Referral Partner Development & Territory Growth (60%)

  • Build and manage a target list of referral partners across hospitals, EDs, community mental health centers, and outpatient providers in Los Angles, segmented by priority and referral potential
  • Run systematic, recurring outreach to existing partners. This means regular touchpoints, not one-and-done visits in order to protect and grow referral volume from your top accounts
  • Prospect and activate new referral sources based on net-new active referral source targets per quarter
  • Build relationships at multiple levels within a health system: discharge planners and case managers day-to-day, with a path to physician and leadership-level relationships over time
  • Re-engage lapsed or dormant referral sources by identifying why volume dropped and rebuilding the relationship
  • Understand the competitive landscape in your market; including where patients are currently being referred instead, and why, and adjust your approach accordingly
  • Represent Amae at community events, provider in-services, and health system meetings to build visibility and credibility in the market
  • Serve as the face of Amae Health in your market and the person providers call first

Warm Handoff & Patient-Facing Support (20%)

  • Personally manage the warm handoff for referred patients by meeting them in the hospital, at home, or in the community as needed to reduce drop-off between referral and first visit
  • Own rapid response for urgent patient situations, coordinating directly with hospital systems, social workers, and discharge planners to move patients into care without delay
  • Be the person a patient or family trusts to walk them through the door, often at their most vulnerable moment

Pipeline Management & Reporting (20%)

  • Maintain complete, accurate, real-time CRM records of every touchpoint, referral, and outcome
  • Own a quarterly market plan with named accounts, targets, and outreach cadence, built in partnership with clinical and leadership teams
  • Report weekly on pipeline health, conversion, and account-level risk

What You'll Bring
  • 3+ years in a quota-carrying, relationship-driven role, healthcare business development, referral/provider relations, medical device or pharma sales, or B2B account management
  • A track record you can point to: a territory or book of business you built, grew, or turned around
  • Existing familiarity with the Los Angeles healthcare landscape (hospital systems, EDs, case management, behavioral health referral pathways) is a strong plus
  • Comfort operating with a number attached to your name, you want to know your target and beat it
  • Comfort sitting with a patient or family in a hard moment 
  • Sharp written and verbal communication; confident presenting to clinical and executive stakeholders alike
  • Reliable transportation and willingness to be in the field daily across the [Market] region
  • Fluency with CRM tools, Google Suite, and Slack 
Compensation & Benefits

Amae Health is committed to fair and equitable compensation practices. Base salary range for this role is $67,500-$94,500 per year, plus a 20% quarterly bonus, tied to referral and conversion targets, and will be adjusted to market cost.
