Job description

Prime Home is looking for shared living provider applicants to place clients who will reside in your home.

Fluent in ASL (American Sign Language) and English


An Extended Family Home (Shared Living Provider) opens their home for a individual with a disability. This position requires an open bedroom and ability to help take care of an individual with a disability. The goal of this service is to encourage and teach skill development so the individual can be as independent as possible. Range of client needs varies between highly behavioral individual to people with higher medical needs.


This subcontracting position is a work-from-home position, where you provide care and support for a person in your own home, as well as provide transportation as needed. This position requires a desire to make a difference in an individual's life. Prime Home is here to assist and support you in the process and follow all State Regulation. SLPs are paid anywhere between $36,000-$105,000 per year, depending on the individual funding received from the state. This is Tax Free Money as it is a protected income by law. On top of this yearly salary, SLPs are also paid $600 Room and Board per month for any individuals living with them to help pay for groceries, rent/mortgage, etc. Potential management of behavioral issues such as aggression, property destruction, etc.


The applicant process is thorough and detailed, so it depends on many factors:


  • Accuracy/Completeness of Application.
  • Scheduling both phone and in person interviews.
  • Scheduling a walkthrough of the home services will be provided.
  • All applicants, SLPs, Alt Care, and non-provider household members over the age of 13 are required to undergo initial and annual background checks, which include Criminal, APS/CPS, and Sex Offender histories.
  • Completion of required trainings (provided).
  • Proof of Auto Insurance and Homeowners/Renters Insurance
