Job description

Springvale Assisted Living is seeking a PART-TIME CMA/UNIVERSAL WORKER. 4(four), 8-hour shifts are scheduled per week (64 hours/pay period). Scheduled hours are 5:45AM - 2:15PM. This position includes every other Weekend and every other Holiday.

Job Title: MED MANAGER/UNIVERSAL WORKER

Primary Function and Relationship to Total Organization

The Medication Manager/Universal Worker provide care, supervision and support for the Tenants at Springvale Assisted Living. Medication Management support under the direction of Delegating Nurse.

Reports to: Springvale Assisted Living Manager or Director of Long Term Care

Essential Duties and Responsibilities

1. Help plan and implement programs of supervision and care for Tenants of Assisted Living:

⦁ Provide assistance with activities of daily living (dressing, bathing, personal hygiene, etc.) or instrumental activities of daily living (use of telephone, housekeeping tasks, laundry, etc.) needed and/or desired by the Tenants and included in the Service Plan.

⦁ Be accessible to each Tenant per CODE ALERT emergency system.

⦁ Monitoring each tenant’s patterns of living, physical condition and/or cognitive/emotional status and report status to the House Supervisor or Director.

2. Housekeeping/dietary functions:

⦁ Escort Tenants to and from the dining room for meals (per Service Plan).

⦁ Set-up, serve meal, bus tables, does dishes, and etc.

⦁ Have knowledge of Tenant’s diet to enable Tenants to stay within a specified diet plan (if special diet is ordered).

⦁ Undertake domestic duties such as emptying waste containers, vacuuming, general cleaning, washing, ironing, making beds and changing linens, dusting and polishing furniture (Per Service Plan).

⦁ Maintain the cleanliness of the non-dwelling units of Springvale including but not limited to offices, lounges, beauty shop, hallways, insides of windows, and etc. as per cleaning schedule.

⦁ Monitor for environment safety hazards, problems or damage to rooms, and any theft.

3. Nurse/Med Manager and social functions:

⦁ Supervise and arrange activities to enhance the physical, social, emotional, and intellectual development of Springvale Assisted Living Tenants.

⦁ Provide companionship, friendship emotional support during Tenant centered daily activities.

⦁ Relate to Tenants, staff, and guests in a courteous and diplomatic manner under all circumstances.

⦁ Provide support to the Tenant’s family members answering questions about the Tenant’s health and welfare and including them in Tenant’s Service Plan meetings if the Tenant desires their inclusion.

⦁ Work to maintain a social model of living/care for assisted living Tenants.

⦁ Assist Tenants with self-administration of medications, including monitoring of medication supplies and documentation OR administer medications as directed in the Service Plan.

⦁ Record in the Tenant’s chart as needed.

4. Facility functions:

⦁ Participates in a team care concept by attending and participating in scheduled staff meetings, in-services, and quality assurance programs.

⦁ Participates in the training of new staff.

⦁ Responds to and acts appropriately in emergency or disaster situations.

⦁ Conducts rounds when the Springvale Assisted Living House Supervisor or Registered Nurse is not available.

In addition to the functions and duties described above, this role may include other responsibilities and duties as assigned on occasion, based upon Humboldt County Memorial Hospital’s needs or requirements.

Minimum Educational Requirements:

Education:

  • High school education or equivalent.

Experience:

  • Prior CNA or Medication Manager experience in Nursing Home or Assisted Living preferred.
  • Med manager certification preferred.
