Location: Skid Row, Downtown Los Angeles, CA


Position Purpose: The Registered Nurse (RN) will serve as a leader within a multidisciplinary care team, delivering professional nursing care to unhoused individuals requiring medical services. Working at the adult residential SUD treatment facility and wellness center in the Skid Row area of Los Angeles, the RN will provide direct patient care, oversee nursing staff, collaborate with providers, and uphold clinical protocols to support patients' recovery and overall wellness. In this role, you will be essential to our mission to provide transformative care to individuals in crisis. Your clinical expertise, leadership, and compassionate approach will promote the health and well-being of our clients/residents, empowering them on their journey toward stability and renewed hope.

About the Crocker Care Campus & Harm Reduction Model:

The Crocker Care Campus is a radical, patient-centered space committed to harm reduction and trauma-informed care. Designed as a welcoming drop-in site for people who use drugs and those experiencing homelessness, it provides a safe environment for hospitality, community, and comprehensive care. Unlike traditional clinics, Crocker operates with flexibility, prioritizing accessibility and support over rigid structures. Our approach acknowledges the complex medical and psychosocial histories of our participants, including individuals who use drugs, engage in sex work, have been incarcerated, or have faced structural racism, poverty, and trauma. We recognize unhealthy drug use as a multifaceted issue driven by systemic barriers and are committed to care that is nonjudgmental, compassionate, and rooted in social justice.

Principal Responsibilities:

  1. Provide compassionate, low-barrier nursing care in alignment with harm reduction principles.
  2. Perform triage, patient assessments, and develop individualized care plans.
  3. Conduct nurse-only visits, including wound care (for complex chronic wounds), medication reconciliation/management, phlebotomy (for patients with challenging venous access), and complex care coordination.
  4. Administer medications and provide essential nursing services per established protocols and standards.
  5. Monitor patients in observation beds, responding promptly to changes in condition and ensuring appropriate interventions.
  6. Offer health education and risk reduction counseling, including safe drug use practices and overdose prevention.
  7. Administer Narcan and train participants/staff in overdose response and harm reduction strategies.
  8. Work collaboratively with social workers, case managers, and harm reduction specialists to address physical health, mental health, and substance use needs.
  9. Support participants in navigating the healthcare system, including referrals to specialty care, housing, and social services.
  10. Recognize and respond to signs of intoxication, withdrawal, and overdose, ensuring timely intervention and support.
  11. Ensure patient care is accessible in non-traditional settings, including drop-in environments and outdoor areas.
  12. Provide guidance and support to Medical Assistants.
  13. Assist in the orientation and development of new staff members.
  14. Conduct training sessions, including Narcan administration and harm reduction best practices.
  15. Oversee patient intake and discharge processes, ensuring all procedures are clearly understood and followed.
  16. Maintain precise and up-to-date documentation of patient interactions and care provided.
  17. Maintain accurate and comprehensive site logs, ensuring smooth workflow and program efficiency.
  18. Work closely with the Program Director and Nurse Manager to support daily operations and quality improvement initiatives.
  19. Partner with the RN Auditor/Compliance team to ensure adherence to regulatory standards and internal protocols.
  20. Perform additional duties as required by the program.

Requirements:

  1. Valid Registered Nurse (RN) license issued by the State of California.
  2. Current certification in CPR from the American Heart Association.
  3. A minimum of three years' experience in a clinical or healthcare setting, with a strong preference for experience in community health or care for vulnerable populations.
  4. Desire to work in a non-traditional clinical setting, including comfort in providing care outside of traditional clinical areas (e.g., outdoor parklets, drop-in spaces, etc.).
  5. A demonstrable passion for providing non-judgmental and compassionate care to patients experiencing homelessness and managing complex, often severe, medical and psychiatric conditions.
  6. Proficiency in conducting comprehensive health assessments, administering medications, and providing nursing interventions.
  7. Exceptional interpersonal skills, cultural competence, and the ability to communicate effectively with diverse populations.
  8. Bilingual Ability (Spanish/English) preferred.
