Job description

Facility is seeking Chief Medical Officer to lead our team - There is an expectation that the CMO will participate in providing care (whether in a clinic or ALF) for 2-3 days a week, with the remainder being used for administrative time. - The CMO will be a member of the Senior Leadership Team responsible for designing, approving and executing the strategy of the company. As our company grows, we anticipate the CMO role evolving, with greater emphasis on administration and management. - You will be leading accountability for performance including: - Leading our clinical team (hiring, firing, performance management) - Aligning with other clinical leaders to define standards and clinic quality program - Ensuring standards are understood and applied universally - Align Provider incentives - Developing a winning culture of accountability - Identifying and removing barriers to performance - Partnering with finance and operations teams to optimize coding performance - You will be responsible for ensuring we support and equip our comprehensive care teams , this may include: - Refining and overseeing our team-based clinical care delivery model - Leading the innovation of care delivery including: The use of hybrid onsite and virtual care; The use of new care coordination models including RPM, CCM and APCM; Integration of care with hospice, home health, specialists and hospitals - Onsite visits to role-model care delivery for complex patients - With support from 3rd party experts, developing world-class clinical onboarding and on-going training programs to equip onsite providers - Partnering with leadership to lower administrative burden on providers - Fostering a culture of collective collaboration among providers and care teams - Partnering with leadership on evolving our risk revenue strategy with Medicare / MA plans - Our Senior Care team deploys an integrated model that includes an onsite care team (typically an NP/PA + RN/LVN) supported by a virtual team of care specialists, therapists, care coordinators and health concierges. - Assisted Living patients are among the most complex, averaging 3x as many ED visits and 6x as many IP admissions as a community dwelling Medicare beneficiary. Furthermore, these patients are administratively very complex to serve - no cars, no cell phones, high levels of dementia and many with powers of attorney. - This is a high growth, fast paced organization.
