For our services, we have been recognized as one of the nation's top 100 performing home health agencies by HomeCare Elite.A DAY IN THE LIFE OF A HI-TECH REGISTERED NURSE (RN)As an RN, you play a key role in providing individualized goal directed nursing care through the use of the nursing process in accordance with physician's orders.Assesses and diagnoses patient and family needs in order to provide quality care to assigned patients.Develops, discusses and communicates a realistic plan of care for each patient, including patient, caregiver in order to address all identified needs.Implements medical plan of care, nursing interventions, and procedures as necessary to ensure the safe effective quality of care for patients.Evaluates a variety of aspects of implemented plan of care including, but not limited to identified problems, care provided and patient's responses to meet patient and family needs and ensure overall quality of care delivered.Maintains up-to-date and accurate documentation of nursing care provided to ensure the effective integration of information for use by the health care team to ensure on-going and continued quality of care.Performs patient and care giver education to ensure informed patient decisions can be made, including, but not limited to, medication side effects, treatment options, discharge instructions, reportable conditions, ranges of services, etc. in accordance with Vermont State Board of Nursing and VNAHSR policies and procedures.Pro-actively advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient/family's needs.Coordinates a wide variety of patient care with physicians, providers, team members, etc. in support of the recommended plan of care to ensure safe and effective patient and family care.Receives monthly (in advance) schedule of patient/patient visits from supervisor. Notifies supervisor of need for changes in a timely manner.Qualifications:Graduation from an accredited school of nursing required. Bachelors of Science in Nursing preferred.Prior clinical acute care nursing experience required.Valid VT Registered Nurse license requiredValid Driver's License required
